Care Coordination Support Specialist :: Responsibilities

Patient Call Handling & Communication

  • Receive and triage inbound calls from patients, caregivers, and RPM vendors
  • Gather patient information (symptoms, concerns, needs) and document in EHR/Care Management Platform
  • Implement standing orders for Hospital @ Home admitted patients
  • Escalate to on-call provider, 3rd party service provider, or care management team as appropriate
  • Ensure 100% overnight calls are answered and thoroughly documented

Remote Patient Monitoring

  • Monitor physiological data via remote portals for abnormal readings
  • Record caregiver-provided manual readings in EHR
  • Apply triage protocols to abnormal readings and escalate when necessary
  • Maintain accurate documentation of patient physiological readings

Medical Chart Review & Documentation

  • Review patient records for new lab values, radiology reports, and external encounters
  • Attach external records to patient charts
  • Identify suspect conditions and document clinical reasoning per coding rule sets
  • Enter suspect conditions into progress notes for provider evaluation
  • Link diagnosis codes to procedure codes as appropriate
  • Track adherence to clinical protocols
  • Review minimum of 25 records daily with accurate documentation

Operational & Administrative Support

  • Complete daily tasks assigned by supervisor/manager
  • Assist in identifying and improving operational processes
  • Support adherence to organizational and clinical guidelines

Qualifications

  • MBBS degree with 2–3 years of clinical experience
  • Excellent spoken and written English communication skills
  • Willingness to work Early Morning Shift and Overlap Shift (IST): 3:30 AM – 12:30 PM and 12:30 PM to 9:30 PM
  • Cooperative attitude and ability to work in a team-based environment
  • Strong Microsoft Excel skills
  • Ability to understand medical information, facility policies, and procedures, and present this information clearly to appropriate personnel

Preference be given to the candidates having experience in healthcare quality operations or project management involving EHR systems

Preferred: Prior experience working with Electronic Health Records
